Dental Healthcare Practice (DHP) Online Membership Terms and Conditions

Plan Payment

To enrol in the Dental Healthcare Practice (DHP) online membership plan, you must pay an initial administration fee, which will be added to your first plan payment. Payment can be made by Direct Debit, credit card, or debit card via Paypal. The plan covers dental services as described on the practice website.

Discounts

Discounts on the monthly membership fee are at the discretion of the Dentist and only apply to group payments made by a single Direct Debit or annual payment.

Exclusions

Certain treatments are excluded from the DHP membership plan, including restorative treatment, purely cosmetic treatment, and any treatment that is not clinically necessary. Additionally, treatment carried out elsewhere that needs correcting, sedation fees, and pharmaceutical items or laboratory fees charged by the Dentist are also excluded.

Appointment Booking

The first appointment under the DHP online membership plan is booked six months after signing up. If you wish to be seen earlier, you can book an appointment separately from the plan and pay for it separately. All appointments must be scheduled following the practice rules and procedures.

Payment Collection

Even if someone else is paying the plan fee on your behalf, you are still responsible for paying the fees. For a patient under 18, the payer must ensure compliance with the agreement. Failure to pay the plan fee may result in the Dental Healthcare Practice taking steps to collect the payment, leading to the termination of the agreement.

Emergency Treatment

The Dentist at the Dental Healthcare Practice will provide dental services but may arrange for a qualified person to perform the services if necessary. Emergency treatment will be discounted for plan members, and members who pay the annual fee will receive one free appointment. It is important to note that this membership plan does not function as a health insurance policy and does not include worldwide emergency dental health coverage.

Compliance

To maintain the benefits of the DHP online membership plan, you must attend the Dental Healthcare Practice for regular check-ups and treatment, accept the advice the treating Dentist provides, and inform them of any relevant matters affecting your dental health. Failure to comply with these requirements may make you liable for treatment fees.

Amendments

The Dentist reserves the right to increase the plan fee once every 12 months and will provide one month’s notice before the increase is applied. The Dental Healthcare Practice may also amend plan benefits by providing one month’s notice. Furthermore, the Dental Healthcare Practice may change the conditions of the agreement to account for changes in the law, including variations in the fee due to an increase in taxation.

Termination of Membership

As a Dental Healthcare Practice (DHP) member, you can cancel your membership anytime. To initiate the cancellation process, you can conveniently do so through your online account on the DHP website or by contacting your payments service provider, such as PayPal.

Cancellation and Refund Policy

If you decide to cancel your membership within 7 days of making the first payment, you may be eligible for a refund. Please note that after this 7-day period, no refunds will be issued for any payments made towards the DHP membership.

It is essential to understand that once the 7-day refund window has passed, you will no longer be eligible for any reimbursement or prorated refunds for your remaining membership period. Therefore, it is advisable to carefully consider your decision to cancel and initiate the process within the specified time frame if you wish to receive a refund.

We recommend reviewing the terms and conditions of your payments service provider for any additional refund policies or procedures that may apply.
